- Send: Paste into the boarding request - mes-demarches, phyto.dbs@administration.gov.pf. When: At least 48 hours before arrival. Fee 1,000 XPF, card link valid 7 days.
- Maritime pet import request (answer sheet). Blank checked 2026-10-01.
- Send: Paste into the pet import request and upload one PDF per document per animal - mes-demarches, zoo.dbs@biosecurite.gov.pf. When: Before you leave the last country. Pay within 5 days of filing.
- Form 211A (legacy, for reference only). Blank checked 2026-10-01. Official source.
- Send: The 2021 paper form, only if an official still asks for it - Do not send it instead of the online pet application. When: The online application replaced this form.
- Rabies vaccination attestation. Blank checked 2026-10-01. Official source.
- Send: One certificate per vaccination, signed and stamped by the vet - Upload with the pet request. When: Before you file the pet request
- Parasite treatment attestation (first). Blank checked 2026-10-01. Official source.
- Send: First parasite treatment, signed by a vet - Upload with the pet request. When: Before departure from the last country, or within the month before landing. Ask DBS which date they will use.
